According to my business’s need , I would like to add 2 new fields named “Evidence File” and “Evidence Photo” for the purpose of asking NRC and government issued document from the user who want to claim his/her biz.

Therefore, I would like to know is there any included functionalities in add-ons, if so plz tell me the step-by-step guidance to me. If not, plz advise me how can i do this.

It is urgent matter. I appreciate ur support.
