I’m trying to build a church directory and have most churches categorized as, for example, Pentecostal (top-level category) and Church of God.

The permalink structure creates URLs like 

mysite.com/churches/city-name/pentecostal/name-of-church

, but I’d like it to create URLs like 

mysite.com/churches/city-name/pentecostal/church-of-god/name-of-church

.

Is there any way to do this?

Because of custom post types and custom location in permalinks, I think only 1 category will be shown in the URL being that the parent OR the sub-category.
