Have you added a map to GD Home top section?
Go to GD > design > navigation and add relevat links to your menu.
Then go to GD > multilacation settings and add the location switch to the menu.

GD > permalinks > choose yourvtype of URL on the top of that page. That should fix your URL and breadcrumbs, and the city only option will only work if you restrict GD to the default country in multi location settings.

I have the setting of “Enable default country” enabled, which only fixes the menu, not slug structure.

The city only URL will only work if you use default country AND region [because there will be usually locations with the same name in more than one region].

So it will apply to listings, but only to locations if using default country AND region.
