I’m sorry I guess those links aren’t showing on home page but please see following-

State link works – http://assistedlivingconnect.com/location/arizona/

Some city links works
http://assistedlivingconnect.com/location/arizona/anthem/
http://assistedlivingconnect.com/location/arizona/chandler/

Most others do not, for example-
http://assistedlivingconnect.com/location/arizona/mesa/
This state/region has records.

BTW, the MAP menu item goes to GD-Home and all places geo-locate on that map.
