Thanks for your help, that does not solve my problem. With that shortcode, you can just show categories name and on the click, it will open that category page.
I was looking for if I can select the category from that and then submit the full form after completing all fields.
Can we show them above the form?

Hi Naveen, I want to show the images within the form. I want to show the name of the category and image as well.
