I see two fields now. It appears to be searching but it doesn’t show anything. When I try to “select business” I box pops up with “Select Business” and there’s nothing to select.

For clarification, the send button does work. I thought it didn’t because it was taking so long to process. And, I didn’t see the CSS for it (it must have been resolved outside of CSS).
The only thing left is the “claim” button is in front of the form and the form doesn’t move/scroll so the fields may be accessible above the keyboard. So, it’s just those two things (unless something can be done about load time).
