I think it is a hosting issue. I am with WPEngine, but am having the same issues with another site that uses different plugins.

I updated to V2 core Version 2.0.0.38. The advanced settings are showing up fine and everything looks like it is working well.

I found where I can hide the country in the Field settings for address, but like I mentioned above, I would like to change the layout.
