Hi,

Yes csv format(field order) should be same as sample and address related fields are required.

Please advise ?

Hi,
It appears to be a GD plugin template engine issue.
I will fix it and it will work fine in next release.

Hi,

Thanks for highlighting this. I will fix it in next release.
Temporally plz using css to hide that specific list item.
Something like
li.(the class on that particular list item)
{
display:none;
}
