CPT File Upload

I’ve added a custom field under my CPT of Locations (placed) for a file upload. BUT it only allows me to upload 1 file. I need to be able to have a list of PDF files for several locations. These are patient forms they will need to click on and download.

I thought about adding up to 8 Custom Fields for this, but I do not want the title repeating. I want this:

New Patient Forms:
Form 1
Form 2
Etc.

Can you please tell me how to do this?
