Default image resists to show

It is the first time I encounter this.
In a CPT, I have loaded 2 categories without issue, they all have default image shown up. Now the third category listings have no default image, I have checked the following:
1. database table, compared listings in same CPT, nothing unusual, with correct category ID etc.
2. checked category setting, same as all others
3. Term Count doneAttached image showing all three categories listings, boxed by category.
Please advise what have I missed?

Thanks for your reply. This sounds like a cache issue on your server/Website. Could you try using another browser? Also, share the URL of the page in question and your WP admin access here in private reply.

So I re-uploaded it to the category and all seems fine now.

Currently it is displayed in descending publish date time order.

Yes, normally only media uploaded through the admin area finds its way into media.Is CPT listing customizable?

Would there be bug in importing process as no show image happened again. I did all routine check and found something in database where feature_image field has un-intentional data. In my CSV there is no such field, like all my other csv, and after importing, it should show NULL value. But this one inserted a YYYYMM in front of NULL value. Could it be an accident insert during processing, this is year and month of when I import.
This happened several times, each time I discover this because of default image not showing. As I mentioned, I do not have such field in CSV, so this field should normally stay NULL.
I am deleting all records for this category and re-import again see what happen. I did this for couple of imports before and they worked in second import with the same csv.
Please advise.

It is not csv issue i believe. Compared data in database, those with image and without has exactly the same data fields, empty fields all with same value as well.
I use browser debug tool and checked that the image path for problem listing showing ../uploads/2019/01/NULL
In database, both showing and not showing default image listings have the same NULL value.
What can you think of the possible issue will be?
