Front End Add Listing Form Doubled on Page

I have two issues.

1) When using the shortcode [gd_add_listing] for specific post types, the resulting form is doubled on the page. Example, on this page: http://scoutjr.com/SITE/add-a-team/?listing_type=gd_teams I’m using the shortcode [gd_add_listing listing_type=gd_teams login_msg=”Please register and login to submit listings” show_login=true] but the form is doubled.

2) I’ve added three custom fields to the custom post type teams, but none of them are showing up on the front-end add listing form, but they do show up on the back-end form. Custom fields are enabled for the custom post type, and I’ve tried every combination of settings with no luck.

What can I do to fix these issues?
