GD Online Demo and "All Locations" page?

    The online demo of GD has a link in the top menu “All Locations” This link then goes to a page that displays all the current locations that can be selected from.
    http://wpgeo.directory/all-locations/

    I would like to have the same page on my site for users to select from. How do I make the same page?

    Please look again at the demo page I am talking about. I really want a full page that shows the locations exactly like you are running in the demo shown at this link:
    http://wpgeo.directory/all-locations/

    The shortcode does not make a page like the demo. It looks like some kind of switcher for a side bar.

    There are 2 shortcodes. 1 to reproduce the menu siwtcher, the other to make a page like the page on the demo.
