    the file you download from the application is correct, you need to change settings in open office when saving.

    Should I use the delimiters for the headers as well?

    Should it be:
    “post_address”
    “123 Elm St”

    with the “” on both?

    You shouldn’t be adding anything in the file… When you save it you have to select those settings.
