Most Locations not working with map URL

    On my site, assistedlivingconnect.com, the city selection for most cities under CHANGE LOCATION menu item mostly returns “no records found”, and some 404. Chandler and Anthem return mapped results. Can’t figure out what is causing this, need some help.
