Multiple Issues with Addons

I am having major issues with several of the addons: (This is a brand new directory, not live yet and with the latest WP version installed)
1). When I try to activate Ajax Duplicate 2.0.0.1 I get the following fatal error message:

2. When I try to activate both the Custom Google Maps 2.0.0.1 and the Recaptcha plugin 2.0.0.1 my entire website locks up, I get a white screen and to clear it I have to thru CPanel and delete both of those plugins. I have downloaded fresh copies of these two plugins repeatedly and it still happens.
