I’ve just checked with a developer and to achieve what you needed there requires customization which falls outside the scope of what we offer for support. But if you think you’re capable of customizing it yourself, copy this template, modify it and use it in theme. “/wp-content/plugins/geodirectory/geodirectory-templates/reg_frm.php” However, you’ll need to write custom code to handle the extra form fields.
