Unable to add Place categories

We have moved to a support ticketing system and our forums are now closed.

    This is a fresh install with no other plugins activated. I get this error message:
    Could not insert term into the database

    What makes it even stranger is that I have done 2 complete re-installs of the site in an effort to fix the problem, and immediately after the install I am able to add a category without problem, but when I check later, it’s missing, and when I try to add another category, I get the above message.

    Can you help?

    I traced this to a problem with a custom theme so all’s well for now. Not sure yet exactly what was causing the problem.
