Map in search page has disappeared

I’m using a Supreme directory. I don’t know what I made but my Map in search page has disappeared. in page divided in half, with a map showing on the right hand side.
Where is this page template to customize?
What could be the problem, maybe some plugin? I have disabled one by one but nothing …

In this page also are two search box, How can I remove one?
