Empty page on clicking 'write a revieuw'

Hi,
I use Supreme directory on zorgactor.be. Two CPT’s where set up:
a) Zorgactoren (places)
b) Evenementen (events)

In the detail page, on Clicking the button “Schrijf een beoordeling” (write a revieuw) I got:
a) an empty page if CPT is ‘Zorgactoren”
b) the expected form to fil in a review if CPT is ‘Evenement’

How can I restore the revieuw form in the detailpage when CPT is ‘Zorgactoren’?
