How to connect new form to "Write a Review" button

    Hi–

    I have learned that users of my website don’t want to publish their names or contact info as they review companies–an understandable concern. Also, I want to prompt them with a set of questions so that they write a thorough review. So! I built a form and put it on a page: https://mortgagejustice.net/review-a-mortgage-lender/ The form includes a series of questions to prompt thorough reviews and makes it clear that I won’t publish reviewers’ names or email addresses. I added this page, with the new form on it, to the menu.

    How can I make this new form the *only* way that people can review a business on my site? Maybe connect it to the “Write a Review” button?

    The goal again–a readable, *very* easy to find form that includes the content on this page: https://mortgagejustice.net/review-a-mortgage-lender/ and make this the *only* way that people can write a review on my website.

    I appreciate any help you can offer on this and thank you for all your help so far. I am a total n00b.
