I’ve just checked with a developer and to achieve what you needed there requires customization which falls outside the scope of what we offer for support. But if you think you’re capable of customizing it yourself, copy this template, modify it and use it in theme. “/wp-content/plugins/geodirectory/geodirectory-templates/reg_frm.php” However, you’ll need to write custom code to handle the extra form fields. Well, I hope it helps and please let us know how it goes.

I noticed that you’re not using the “Beaver Builder” plugin on the homepage anymore. Probably, you’ve temporary removed it while waiting for my response. Please explain to me a little bit more regarding your issue and I’ll try my best to help you with this.
