I don’t think thats the problem.
But the file path seems like using the absolute url in your website instead of relative url.
Maybe you imported or migrated with full url.

We have not released any new version since the first post.
However we often link the user to download the dev copy.

I believe at this moment that repo is stable.
If you have never downloaded the plugin from that page, then you are using the beta version given by paolo. In that case you can try using the dev version and check whether it fixes your problem or not.
If you have recently downloaded from that page, then i believe the recent changes broke your site.
I’ll debug your issue tomorrow and give you proper solution.
Did you use our development version of UsersWP in your site?
I pushed the unstable code everyday. If you use the dev copy in your site most likely it won’t work.
You have any idea when exactly you updated UsersWP?
Besides,the form builder does not keep changes when trying to set the filed username inactive, for example.
