After Clicking "Claim Listing" Form doesn't Load

    Hello,

    After clicking the claim listing button a light box opens and looks like it is loading but never actually loads a form. Here is a link to the listing page http://myccl2.wpengine.com/directory

    Any help would be appreciated.
