Archive pages for custom post type not working

We have moved to a support ticketing system and our forums are now closed.

    To see the issue we are having, please load the following page:

    https://therapyforblackmen.org/therapists/new-york/brooklyn/seneca-williamslmhc/

    Then, if you click ‘Brooklyn’ or ‘New York’ in the breadcrumb, you will see a page not found error. ‘Therapists’ does load a page however.

    Please help us figure out how to fix this. Resetting permalinks had no effect.
