Cities with same name

Hi,
I have some listings that are in different states but they have the same city name.
This seems to be causing an issue with filtering: When I want to filter by location, it includes listings from the other state with the same city name.
Also, there is a listing page for the location. In my case, /accountants/covington, and this page lists the accountants in all Covington locations, regardless of states. It seems like the location page should be the combination of city and state, or state is before city in terms of location drill down. Is this a settings issue?

-update – I think I fixed this. I needed to first change the multi location settings, save them, then change them back and save again.
