Custom Field Types To Use

Hello

Please I am trying to create a form that is in the format below:

QUALIFICATIONS
MSC – Yes/No
MBS – Yes/No
MMN – Yes/No
CPA – Yes/No
BBA (If yes, please specify ________) Yes/No

All of these are supposed to be under the “Qualification” that will be displayed on the More Info tab. Am I supposed to create each of them one-by-one using “select” or can this be done in another way?

Also very important is if a conditional option like (if Yes, please specify) can be added using “select”of other means?
