The theme used on that site is a customized Kleo theme. It is not available as is, the theme is customized to combine KLeo and Supreme.

Hi,
I’m trying to create a layout similar to Arbolife using Kleo and Supreme Directory.

How can I tell what options are part of supreme theme/GD settings and what was customized?

For example, on listings page https://www.arbolife.com/en/stores/health-food/, Arbolife displays categories and address for each listing. Is this a setting that can be turned on/off?

Those options are not theme dependent. Please review https://wpgeodirectory.com/docs/core-place-settings/#show
You will need to go to GD > Place Settings and for each custom field you want to be displayed as part of the list view, select “Listings page” at the “Show in what locations?” setting.
