Category is required, so, you could set category as a select field, and then just create one category to be applied to all listings.
The options on the permalinks page are specific to the detail page, so you can create a URL structure like you mentioned /firm/state/city/post-name/

Category will still be included on archive pages. You can remove category from the display so that users do not end up on those pages.

This depends on what data is being returned by the Map API. If it returns California, then you will need to use the options in the Locations tab to maintain the regions.
