They cannot be the same page, but you can certainly design them with similar or the same elements.
As a reminder, an archive page only shows one post type at a time.

The options on the permalinks page are specific to the detail page, so you can create a URL structure like you mentioned /firm/state/city/post-name/

Category will still be included on archive pages. You can remove category from the display so that users do not end up on those pages.
2. CA vs California
This depends on what data is being returned by the Map API. If it returns California, then you will need to use the options in the Locations tab to maintain the regions.
