The problem was that the Street address was put in quotes, so that tripped the CSV.
Once the quotes are deleted, the CSV works fine.1. See image.

The first wrong field is actually the address field. It contains street address, city and region, and that is why everything shifts up two columns.

If the site is for restaurants only, you could rename places to restaurants, and then create categories for Italian etc. That’s all you would need to do.

You only uploaded the ZIP file to the themes folder, instead of extracting on your computer and overwriting all the files in /wp-content/themes/supreme-directory

If you overwrite a theme using FTP, there will be no update notice, because it is updated by overwriting.
All I can think of is that you did not overwrite all the files, that can only be checked if we have your FTP details.
Child themes do not get updated normally, but Supreme is still in beta.1. Add a link to Find a contractor at Appearance > Menus > Top (Top Navigation)
