The quota is set on the key, not in GD.
HTML is allowed, let me flag this for the developers about the help icons using FA JS 5.5, and the field values for %%item%%.
Google changed their policy and requires billing accounts now. Without billing quotas are set to “1” map view, which is easily exceeded.

If it says “exceeded quote” then it will not work. You will have to set the quotas higher, which is usually done when a billing account is added.
We can take a look, just write back with a private reply including your site login URL and WP Admin credentials so someone on the team can take a look at the settings.

What you want to do is customize your site with CSS. You will need to create the CSS yourself or hire a developer to do it for you.

You might consider trying out GDV2. It may be easier to achieve what you want with V2.

You can try clearing cache, the key can get stuck in cache for not logged in users.

Try setting Max sub-cat depth (max_level) to 1. I think 0 is unlimited. (The instructions might could use more words/clarity).

Yes, but these are settings based on your theme as the styles are inherited from your theme.
For example, if you are using a GD Widget in the sidebar, the widget has an option to choose the heading type for the title, h3 or h4 etc.
You would then adjust the style of h3 on your site in the theme settings to change those title properties.
