Make sure you are using the correct version.
Are you running GDV2 with the V2 version of Directory Starter?Please share a link to the site so we can see the issue, and WP Admin credentials in a private reply if we need to check the settings.

If you are currently using GD Output Locations, then, first find the address custom field in your cpt at
Places – Settings – Custom Fields – Address – Show in extra output locations -> Remvoe “Detail sidebar”
Then visit your widget settings and add GD Post Address widget. With that you can use the advanced options to set the template for the address format.

There are examples here for badges that show for claimed, and badges that show for unclaimed.
Whatever they show for, they disappear if that case is no longer true:

Badges can be added to the Details template, archive item template, or as details page sidebar widgets.
