Do you have a sidebar showing on the detail page now?
If not you can try adjusting the page attributes on that page to pick a template that shows a sidebar.Then place the GD Output Location widget into that sidebar.

If you have pricing manager installed then yes you will need to choose a cart. Otherwise you should uninstall pricing manager.
Can you share a specific example?
In all cases the default radius is applied, so, if you have a listing within say 200 miles and search a term from the title and search IN OHIO, and the listing is within 200 miles of the lat lon of OHIO you will see it in the results.
How is the location page looking? Is it showing the locations? Are you trying to make a city or state specific directory?
