All GD elements are available through the Shortcode builder or as widgets.
Try adding the GD Ninja form widget to the sidebar used on your detail page. GD Ninja Forms it is called.

You will need to download the other Location Manager file for V2. Use all matching versions for V2. For Pricing and Payment Manager choose V2.5+

yes, try different configurations for the permalinks, and resaving them, to address the 404 issue:

For the sidebar, GD doesn’t have a sidebar, it uses your theme sidebar.
