The claim button will only show on listings that don’t have the claim button checked.
I do not see blank forms. I don’t know what you mean with that but send a screenshot if it happens again and we will check it out.You can still use the “GeoDirectory” template for the details page and create the header exactly the way you want.
Try making your permalinks more complex when you resave them as described here:

GD seems to be working, but, yes, the setup wizard shows blank.

The developers will take a look, but they will probably need FTP information to debug, so please share that if you have it.

We plan to update the Yoast breadcrumb integration, it is on the list of improvements, but no timeline yet.
I understand what you mean and have made a note of that request for consideration for future versions.
