You can only filter by users location on the location page or the search results.

to fix the missing column I suggest to open a new sample file and copy + paste each column from your file to the sample file.
the author id of the admin is normally 1 http://guides.slidervilla.com/2014/04/find-user-id-author-wordpress/
the free plugin allow 1 city only. It doesn’t matter if they are adjoining or not, if you want to add listing to a place with a different name you must have the location manager installed.
post_id must be blank to add new listings.

You have the following errors:
1) post_id should be empty (you added the listing title there)
2) All listings should have a title. (few of yours don’t)
3) must put author ID in post_author column
4) all listings must be within the default locaiton unless you install the location manager add-on
5) all default columns must be there even if empty. You removed some from the sample CSV file.

the widget is showing in your code, but empty.
There must be something wrong with your events categories. I suspect the problem is using underscores in slugs, but I can’t be sure.

Yes you could achieve that with pricing manager. Exclude the categories you want from the free price and create a price for each category that you want to charge for.
You can use Stripe to get credit card payments directly on your website.
