We are working on a new “Address Element” that will be released soon. With that you can adjust which parts display.
Until then please use the chrome inspector to create CSS to hide items.

You can adjust the options for the GD Ninja Forms widget to output as text.
For each item you adjust, try opening the advanced options. They are under the blue button.
For fixing styling issue you will need to write the custom CSS yourself or hire a designer.
