For examples, Places is a CPT, so you could go to
Places, Settings, General, Show Advanced, Disable location
Yes, that is for micro-data support from schema.org, making listings “machine readable” by matching the listing schema to published schema on schema.org.
You will need the Custom Post Types addon. Once that is installed, to remove the physical location, you go to
CTP, Settings, General, Advanced, Disable physical location
That will completely remove the address field. You can add back a text field called street address if you still want an address on the listing but want to be rid of maps and other GD integrations related to the location.
Each category requires a map icon, yes.

WP now features an “Additional CSS” option in the customizer
Open Appearance, Customizer, Additional CSS
You can place all custom CSS there.
This feature is built into GDV2, such that the shortcode builder allows the choice of the default post type. Consider upgrading:

I think what you are looking for is the advanced settings under
GD, Settings, Locations, Advanced
There are two new settings for V2. What you are talking about is the default behavior, but you can change it with the settings for:
Archive urls
Archive urls on Details pageGive those a try and see if it is what you are looking for.
Use the shortcode builder to create the gd_listings shortcode you need and be sure to uncheck “Enable Location Filter” in the advanced settings.
In most cases this is caused by bad location data in the dummy data (incomplete default location, perhaps).

the location slug in the GD Permalinks is slightly misleading. It says it is deprecated but that leaves it to you to know the location page slug which is /sams-surveyors/
So your location pages are /sams-surveyors/united-states/florida/bradenton/
The detail pages are fine now and so are listing pages at /surveyors/united-states/florida/bradenton/

UsersWP does include a login form, but it’s best to choose one or the other, BP or UsersWP.
Yes, you have to enter the listings into the site before they will appear on the site.
