1. I think you keep confusing the detail page for ONE listing with the category/listings page for ALL listings in one category. I think Paolo gave an answer guessing what you meant. I still do not know what image you want to replace either. There is no map image on that page. You really have to give us the proper URL and a screenshot or something.
2. The default image of a category is only for displaying on the category/listing pages, like http://www.supremecityguides.com/events/miami-fashion-events/
If you want to display an image on the detail page, you have to add it to the listing at http://www.supremecityguides.com/wp-admin/post.php?post=110&action=editThere is a bug for people that have already upgraded to WP 4.5. Until that bug is fixed, category images and icons cannot be changed. An update is being worked on.

One listing onlyIf you want it to return http://www.supremecityguides.com/events/miami-fashion-events/miami-fashion/ to what it was, then change the slug of the listing back to match the slug of the category, and you no longer will have a category page for other listings in the /miami-fashion-events/ category, because the category page will be broken.

As you can see there are no images added.If you want to change the category image, go to http://www.supremecityguides.com/wp-admin/edit-tags.php?taxonomy=gd_eventscategory&post_type=gd_events and edit the category.
2. That is because you used an “icon” for the category that is bigger than the max 50 x 50 recommended size. An icon is a small image.
3. No, you cannot. Because that link is for ONE listing, not a category pages showing ALL the listings in that category.
They are WP categories, not GD categories.
