Some countries and regions share city names, so if you have more than one country and region, you need the full location, or things will go funny.
Can you please post your WP admin details and FTP details too, thanks. We need to have a closer look.

The claim listing form cannot be linked to, it is a popup form.
You can just created a custom link to ADD MY BUSINESS in the menu, linking it to
/add-listing/?listing_type=gd_place [adjust if required]. That is the easiest way.No need to set them up. Add a GD > CPT Categories widget somewhere and you will get links to every category page.

Did you change your site to HTTPS? It may be that you did not authorise the HTTPS URL in the Google Map API.Your post was flagged as spam, probably the reference to theme forest with some spruikers language 🙂

You do not have any Custom Post Type, not even Places. Do or did you have any listings?
Something must have happened to delete/hide all CPTs and listings.
Do you have a backup?
It cannot be GD Booster, it is not activated.Can you explain what links you want to add to the menu, and then post your URL and WP admin details in a private reply and we will have a look.

In the location pages, that happens automatically:

You should not include a location map on listings pages, see https://wpgeodirectory.com/docs/widgets/Home/Location maps are for location pages, and Listings map are for Listings and Search pages.
