your locations are all wrong. They must be in the same format as data sent from Google Maps API.

Boston, Boston, MA, United States should be:

Boston, Massachusetts, United States

Charleston, Charleston, SC, United States should be:

Charleston, South Carolina, United States

Providence, Providence, Rhode Island, United States should be:

Providence, Rhode Island, United States

Once all this is fixed, the system should start working as it should. Best way to do this is to correct one listing per location so that the correct locations are created, than merge the correct location wit the wrong locations in Manage Locations to adjust all other listings automatically.

I see your website is not indexed by google yet (at least not all pages) which can be the reason why they don’t serve ads yet. They are probably still gathering info to serve ads that are targeted to your website content.
