1) there are several things wrong here. 1st and most important, Studios, Classes and Meetups CPT have no categories that are mandatory for all listings.

Some of the listings (dummy data) are entered for Fort Lauderdale, but the marker is placed over Key Biscane, so probably there is discrepancy between address and lat and long coordinates.

Please add the CPT categories and correct all listings making sure the markers are in the right position compared to the address and everything should start working as it should.

2) That’s because there are no listings for Classes and Meetups. Once you’ll add some listings the CPT names will appear in the dropdown.

3) it autocompletes only if the city exists in your DB (see image attached)

4) with this CSS in GD > design > scripts > custom css, you can hide them from the detail page:

.sd .sd-ratings {
    display: none;
}

Remove it once you want to have them back.

5) Latest version is 1.0.4, you have 1.0.3.

6) Yes you can hide the keyword field,

7) Location will remain set only while visiting Directory pages, if you visit a non GD page, the location will automatically reset.

that selector is not for catgeories, but for Custom Post Types. To place a category selector instead, you would need to customize the code inside the the search form template.
