If you remove the original records and only try to import new records, do they all fail, then?
I do not see any other obvious problems with the sheet. All the categories exist? New locations are allowed?
My concern is the columns are out of order. Did you try our recommended procedure?

Edit the listing and mark “Has franchise” then save.
View the listing again and the GD Author Actions should show an “Add Franchise” link to add a franchise.GDV2 is a completely new plugin in many ways, and the elements like GD Categories have all new markup.

This reply has been marked as private.Please share a link to your page and we will check that out. This isn’t a known behavior on an archive page. There is a similar warning from WP whenever leaving an unsaved page, though, that is expected.
Upgrade to GDV2 and use the 3rd party slider in the Archive item template

GDV2 uses the widget areas that come with your theme. It does not install additional widget areas for you.
GDV2 will Geocode. Consider switching to GDV2!
