If you go to
GD , Settings, Import/Export
and then export the places CSV, that’s what it ought to be compared to. They have to match exactly.
The “Sample” file is not the same as the export, yes, you will need to use the export, not the sample.
The columns must match exactly.

Instead of setting the closed time like 0900-0400, you could set Mon 0900-2400, Tue 0000-0400.

We have put in a task to look at the issues with map icon styling in the gd_categories as used in the Supreme featured aera. This is a complex widget that has undergone many changes in V2 and Supreme v2 specifically. We will report back here with an update about it the improvements.
For the home page sidebar, have you tried changing the page template?
