Cannot figure this one out really. There must be one of your plugins interfering maybe, I suggest to disable any caching (WP Performance Score Booster?) and see whether that fixes anything. You have caching enabled in your config file too.
Are all your GD plugins the latest? Mixing and matching plugins will lead to errors.
