if you see different things when logged in vs logged out, that is 99.9% a caching problem.
If you are using a caching plugin different from GD Booster or WP Super Cache you’ll need to delete its cache and uninstall it.
If the caching is server based, you’ll need to disable that cache too.

As far as I know only a custom jQuery validation would enable you to do such thing, however I asked to the developers to let us know if there is any other way in their opinion.
They’ll let us know asap.

I see multilocations working fine both in front and back end. Why do you say it doesn’t work in front end?
You selected default country and region and only multi city, so you can only add listings to cities of the state of Pennsylvania, United States.

as far as I know/remember, if you enable the permalinks option “Add location slug in listing urls” the listing will only work with the location in it and this never changed.

The “icon” must be transparent and the background must be only around it. Also it must be the exact same size as the default:
