Enfold is a third party theme, and also, we only support non-modified GD products.All GD styling is applied correctly to places. It is only the theme CSS that is missing, but we cannot help with that.

There are no GD issues. The issues you have are with a non-GD theme.

Delete all Places and Place categories and empty Places trash. Then the Places will disappear from your site.
I suggest you install the Location Manager. Then you will have an option for each CPT to completely turn off addresses, and the NEAR search field will just disappear.
ZIP the file or upload to your site and then link back to it. Thanks

When you say “I am using a custom name for the theme” do you mean you created a child theme?
Or did you rename the Enfold parent theme? That should normally not be done, because that can break a lot of connections between files.
If you are using a child theme, then it does not seem to have a style.css file, or you have renamed it, which again may be the problem.
Or that stylesheet may have broken the links to the enfold style sheet.Try undoing all file name changes and see if that fixes it.

Yes, it is the update of the plugin that is resetting that setting, and that is why you get the notification.
