Hi Daniel,
custom fields must be created in advance and you use the html variable in your CSV file.
The CSV file to be used, must be our sample CSV file. You can’t upload just any kind of file format.
The best way to have a properly formatted file is to create at least 1 listing with all custom fields from the back end. Then export the CSV from the Import / Export page.

ask them why saving a form input with value=’VARCHAR’ doesn’t work while if I change the value to VARCHARx it works.
Probably they set some strange security measure on the server.
If you copy Stiofan’s post and they check the jsfiddle they will understand.

With the image of your choice and make sure to keep a back up to replace it after every update.
I asked to the developers to see if there is a hook that would allow to modify it without having to worry about updates, because with CSS it is not possible.

at the moment this is not possible without a customization. Probably with the Widget logic plugin you can do something like that.

lat and lon are mandatory fields.
That website is not that difficult to use. Let me explain:

1) in the input field you copy the list of your addresses
(just copy and paste from your CSV)
2) click the geocode button
3) Copy the lat and lon coordinates from the Output field.
4) Paste the in your CSV.
